Patient tracking form (20KB) – I’ve created a patient tracking chart for medical students that includes admissions information, daily labs, and medications. In the future, I hope to add an editable version.

EBM Calculator (47KB) – I put together an Excel chart that can be used for making biostatistical calculations of sensitivity, specificity, predictive values, and 95% confidence intervals. It can even make a data plot of pre-test versus post-test probabilities.
